April 24,2023 Hi,I am M.praveen kumar (roll no 63)5th semester medical student.This is an online e block to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio A 48 year old male came to OP with severe vomitings. Chief complaints:- The patient complained of severe vomitings from the past 4 days History of present illness:- The patient was apparently asymptomatic 1 month back.Then he developed neck and back pain  for which he went to a hospital. Then renal function tests(RFT) was done and patient was told to undergo  dialysis.patient then ignored and didn't undergone dialysis.4 days back patient had few episodes of vomiting after eating ,he then visited Kims hospital where 2 rounds of dialysis was done.  History of past illness:- The patient is  a known case of diabetes. Kept on medication - for 4 years. Personal history:- Diet- mixed Appetite- decreased  Sleep- normal Bowel Movement’s - regular Addictions: alcoh

April 19,2023 Hi,I am M.praveen kumar (roll no 63)5 th semester medical student.This is an online e block to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio A 35 year old female came to OP with fever and weakness  Chief complaints: The patient complained of severe breathlessness and fever since 1 week History of present illness The patient was apparently asymptomatic 3 months back.Then she developed fever and chills for which she went to a hospital where she was diagnosed with mild grade anemia then she used medication for 1 month. Then 1 month back the patient developed heavy fever and weakness and dyspnea on exertion.then she came to kamineni hospital  4 days back where she was given 2 units of blood and a unit of iron,and was kept on medication  History of past illness: The patient is not a known case of diabetes,asthma,epilepsy and hypertension  Personal history: Diet- mixed Appetite- normal Sleep- decreased

A 30yr old male came to the hospital with complaints of weakness  Chief complaints: The patient came to the hospital on tuesday with chief complaints of weakness and shortness of breath History of presenting illness: The patient was apparently asymptomatic 1yr ago then he visited a local hospital with complaints of weakness and pain at the lumbar region,then dialysis was recommended at the local hospital,for dialysis he visited Gandhi hospital,medications were given 6 months and told to stop alcohol and meat.he took medications for 1 month and and then stopped,he also started consuming alcohol.Then he visited Kims hospital 6 days ago with complaints of shortness of breath. Negative history: Fever- No history of fever. Shortness of breath: history of shortness of breath
